As we work to make becoming an AngelFest vendor more efficient, we're now offering 2 ways to submit vendor applications!

  1. You can use the traditional approach by downloading a PDF form to print, fill out, and send with your payment by clicking on this link: PDF Form

  2. Or you can use our new paperless option by completing the form below and submitting your payment through  The paperless option will generate a confirmation email to know when the form has been received, and we will contact you by phone or email to let you know once your application is accepted.
